Position Summary

The Clinical Manager/Director will provide direct clinical leadership, operational oversight, and quality assurance for a high-performing team of Caregiver Advocates, Caregiver Specialists, and Senior Caregiver Specialists. This role is responsible for supervising remote clinical teams, driving performance on key metrics (KPI/OKRs), and ensuring adherence to evidence-based standards of care within TCARE’s caregiver engagement model.

The ideal candidate brings a blend of clinical expertise, strategic thinking, remote team leadership, and process optimization skills. They must be an effective communicator and a collaborative leader capable of aligning frontline execution with TCARE’s broader clinical and organizational goals.

Key Responsibilities

Clinical Team Leadership

  • Lead and manage a team of in-person and remote Caregiver Advocates, Specialists, and Senior Specialists.
  • Provide coaching, feedback, and performance management to ensure high-quality caregiver engagement and retention.
  • Partner with VP of Clinical Strategy and Operations to support staff development and licensure supervision where appropriate.
  • Model a trauma-informed, culturally responsive approach to care and supervision.

Operational & Performance Management

  • Monitor, report, and improve team performance against KPIs including:
  • Weekly call volume and case activation targets
  • Active case management ratios
  • Care plan completion rates
  • No-show and contact success rates
  • Caregiver retention and engagement
  • Collaborate with the VP of Clinical Strategy and Operations to align day-to-day activities with organizational OKRs.
  • Use data and dashboards to drive performance improvement, flag trends, and propose solutions.

Clinical Quality Assurance

  • Ensure clinical documentation and care plans meet internal standards, TCARE protocol, and payer expectations.
  • Lead quality audits and facilitate feedback loops to improve caregiver experience and compliance.
  • Participate in the development and deployment of SOPs, workflow enhancements, and training materials.

Reporting & Data Utilization

  • Prepare weekly and monthly reports on team productivity and outcomes.
  • Leverage case stratification and burden scores to support staffing alignment and strategic decision-making.
  • Support the development of insights for partner reporting, care model evaluation, and continuous improvement.

Cross-Functional & Strategic Support

  • Serve as a clinical subject matter expert across product, data, and client success teams.
  • Represent clinical leadership in external presentations and partner meetings as needed.
  • Support organizational initiatives related to caregiver assessment, care planning, and population health strategy.
