TCARE® Specialist- Care Manager

The purpose of this role is to assess a family caregiver’s journey utilizing the TCARE Family Caregiver Support Software and create individualized care plans that address burnout, stress, depression and more. You will then provide linkage to appropriate community-based programs and resource connections throughout their entire journey.

Essential Job Functions

  • Conduct full TCARE assessment with family caregivers by phone
  • Create care plans according to training received through TCARE Certification process
  • Link caregivers with social services programs and resources that provide positive outcomes to identified burden and stress measures
  • Conduct on-going follow up assessments and care plan alterations to promote continuity of care and ensure ongoing positive outcomes
  • Foster good judgement for making outside referrals when risk and danger is determined as outlined in care management ethics
  • Participate in case meetings as needed
  • Must be able to travel out of town for training and staff functions when needed

Customer Success

  • Ensure contact is made with caregivers in timely manner as set out by protocol
  • Ensure care plans and follow up assessments are completed in timely manner as set up by protocol
  • Educate family caregiver on the value of each service that is offered under the care plan
  • Strong listening skills with the empathy and compassion in your discussions

Department Operations

  • Maintain records of case notes and actives for each client
  • Assist with national database of services and resources to support family caregivers
  • Provide weekly feedback to stakeholders
  • Provide weekly case status reports

Required Qualifications:

  • Bachelor’s degree in social sciences, gerontology, or a related field
  • 2 + years’ work experience in community-based services, public health, or related field conducting assessments and making referrals
  • 2 + years current case management experience
  • Bilingual Preferred: English/Spanish
  • Outstanding communication skills, both written and verbal is essential
  • Excellent organizational and time management skills
  • Basic knowledge & comfort working with standard software packages
  • Passionate to learn and practice new skills and protocols
  • Experience working with older adults, IDD community or family caregivers is desired
  • Ability to work independently and meet job work expectations and case loads
  • Must have strong empathetic and compassion with the ability to handle delicate conversations
