About the Role:
In this role you will provide virtual medical nutritional therapy to Swap patients utilizing your individual expert skills while also meeting Swap’s high standards for care and counseling. We believe in bringing clinical nutrition guidelines to life through realistic recommendations using personalized grocery shopping tips, food options, products, and recipes based on their nutrition needs, lifestyle, and preferences.

We pride ourselves in building long-term relationships with the people we work with by adapting with patient progress and offering customized care for each individual. You will match with your patients and be their point person throughout their entire journey, meeting regularly through 1:1 sessions via phone or video call.
Key Responsibilities
Personalized Patient Counseling - Provide personalized nutrition counseling to patients utilizing motivational interviewing techniques.
Passion for Food and Nutrition - Expertise and comfort talking about food recommendations, food products and how they relate to a patient’s nutrition care, tradition, and lifestyle.
Organizational Knowledge & Judgment Demonstrate knowledge of the standard operating procedures, processes, and equipment necessary for the role of Swap Health Dietitian. Recognize and resolve potential issues using sound judgment to determine the right course of action. Involves others in the decision-making process as needed.
Skillset: Possess knowledge and expertise to effectively work with a large and diverse patient population. Demonstrate interest in completing assigned training to obtain new skills and/or proactively masters new competencies either independently or as part of team initiatives.
Quality of Care - Demonstrate the highest quality of care for patients; going above and beyond to provide the best patient experience.
Productivity & Dependability - Demonstrate effective time and schedule management; ensure assigned tasks are completed efficiently. Adhere to the set organizational schedule; displays commitment to delivering the assigned work on time and at expected levels of quality/output.
Communication - Exercise effective and appropriate verbal and written communication and listening skills. Remain accessible and available to other employees as needed.
Initiative: Demonstrate resourcefulness, versatility, and teamwork qualities necessary to conceptualize and carry out additional duties. Has a self-starter attitude to solve problems for the care process and patient journey.
Patient focus - Maintain high standards of patient care and satisfaction. Actively solicit and act upon provided patient feedback. Engage patients in Swap services and foster long term relationships with assigned patients.
Qualifications
- Bachelor of Science
- Active Registered Dietitian (RD) or Registered Dietitian-Nutritionist (RDN) credentialed by Commission on Dietetic Registration
- Active license in the state of North Carolina and/or Ohio (Licensed Dietitian)
- 3+ years experience with 1:1 patient counseling/coaching with expertise in motivational interviewing strategies
- 2+ years experience working in a remote or virtual setting delivering care via telehealth
- Experience and comfort discussing food products, label reading, and navigating online products to make recommendations aligned with patient lifestyle, budget, and goals. Retail experience is a plus!
- 2+ years working with electronic health records
- 1+ years experience with counseling low income or underserved populations
- 2+ years experience working with patients managing chronic conditions such as type 2 diabetes, chronic kidney disease, and/or heart disease
Benefits, Compensation, & Hours
- $72,500/yearly
