The Role:

The Certified Diabetes Care and Education Specialist provides evidence-based education, coaching, and care coordination for individuals living with prediabetes, type 1 diabetes, type 2 diabetes, and cardiometabolic comorbidities. This role delivers personalized medical nutrition therapy (within scope), technology training based on equipment, behavior-change support, and clinical monitoring of patients with pre-diabetes requiring support.

For those with other clinical conditions, this position works in close collaboration with physicians, NPs, MAs, CHWs, and other members of the care team. The CDCES ensures high-quality self-management education and support that improves management of hypertension, hyperlipidemia, diabetes and obesity, while reducing complications, and enhancing whole-

person health.

Key Responsibilities:

• Conduct comprehensive assessments, including medical history, psychosocial factors, lifestyle behaviors, medication adherence, and social determinants of health.
• Deliver individualized and group-based Self-Management Education and Support consistent with evidence-based standards.
• Provide tailored education and coaching on:
• Nutrition, meal planning, and glycemic impact of foods.
• Physical activity guidelines specific.
• Preventing and managing hyperglycemia.
• Reducing cardiometabolic risk (lipids, blood pressure, weight).
• Integrate data from RPM if necessary
• Interpret biometric data (e.g., average glucose, TIR, A1C trends, activity, weight) and escalate clinical concerns according to protocol.
• Collaborate closely with the interdisciplinary team to align treatment plans.
• Document all patient interactions, education plans, and outcomes in the EMR following regulatory and organizational guidelines. Support quality-measure performance and document all follow-ups including biometrics
• Contribute to the design and continuous improvement of education programs and workflows.
• Promote culturally responsive care and address barriers to access, affordability, and adherence.

Required Qualifications:

• Active certification as a Certified Diabetes Care and Education Specialist (CDCES)
• Bachelor’s or Associates degree in related field.
• Minimum 2–3 years of experience providing diabetes education or direct diabetes care.
• Demonstrated expertise in DSMES, and evidence-based self-management.
• Competence with digital health platforms, telehealth, and remote monitoring tools.
• Strong knowledge of ADA Standards of Care, ADCES guidelines, and cardiometabolic risk-reduction strategies.
• Exceptional communication and motivational interviewing skills.

Preferred Qualifications

• Experience working in virtual, hybrid, or outpatient population-health programs.
• Familiarity with social determinants of health and culturally responsive diabetes education.
• Experience in value-based care, chronic disease management programs, or employer/payor environments.
