The Role:

The Health Coach plays a central role in supporting patients across nutrition, stress management, anxiety reduction, brain health, physical activity, and biometric monitoring. This role provides evidence-based coaching, personalized behavior-change support, and ongoing accountability to help patients achieve measurable improvements in whole-person health. The ideal candidate has formal training, certification, and experience in lifestyle medicine, nutrition, behavioral health, and digital health tools.

Key Responsibilities

  • Deliver 1:1 and group-based coaching using evidence-based behavior-change frameworks (e.g., motivational interviewing, CBT-informed strategies, and trauma-informed approaches).
  • Conduct comprehensive intake assessments covering nutrition, sleep, stress, mental well-being, physical activity, and lifestyle risk factors.
  • Develop individualized care plans aligned with care goals
  • Provide coaching in:
  • Nutrition and meal planning (with emphasis on cardiometabolic and brain-health–focused nutrition).
  • Stress reduction, mindfulness, anxiety-management techniques, and resilience-building strategies.
  • Physical activity recommendations tailored to patient ability, goals, and comorbidities.
  • Brain health support (sleep hygiene, cognitive habits, lifestyle approaches that support cognitive longevity).
  • Monitor patient progress
  • Interpret biometric, behavioral, and self-reported data to adjust patient plansDocument interactions in the EMR and maintain high standards of care coordination.
  • Support digital-health engagement, including use of mobile apps, wearable integrations as needed, and patient-facing digital resources.
  • Maintain confidentiality and follow all HIPAA and organizational compliance guidelines.
  • Contribute to the design and continuous improvement of education programs and workflows.
  • Support quality-measure performance and document all follow-ups including biometrics

Required Qualifications

  • Bachelor’s degree in Nutrition, Exercise Science, Psychology, Public Health, Health Education, or a related clinical field.
  • Certification as a Health Coach, Nutrition Coach, or equivalent (e.g., NBC-HWC, ACE Health Coach, Precision Nutrition Level 1/2, ACSM, IIN).
  • Formal training in behavior-change science, motivational interviewing, and coaching psychology.
  • Minimum 2–3 years of professional coaching experience in health, wellness, or clinical settings.
  • Demonstrated experience working with patients managing cardiometabolic risk, stress/anxiety, or lifestyle-related conditions.
  • Experience using biometric tracking tools and interpreting health data to guide coaching.
  • Strong communication and rapport-building skills with diverse populations.
  • Comfort working in a technology-enabled clinical environment (telehealth, digital platforms, RPM tools).

Preferred Qualifications

  • Experience in a multidisciplinary or value-based care setting.
  • Familiarity with cardiometabolic, brain health, and longevity-focused lifestyle interventions.
  • Training in mindfulness-based stress reduction (MBSR), CBT skills coaching, or integrative mental well-being approaches.
  • Experience with culturally responsive care and social-determinants-of-health awareness.

Success Measures

  • Patient engagement and satisfaction.
  • Improvement in biometric and behavioral outcomes (e.g., activity levels, nutrition adherence, stress scores, blood pressure, glucose trends).
  • Contribution to care-team coordination and clinical throughput.
  • Adherence to documentation, quality, and compliance standards.
