Job Summary:
We are seeking a knowledgeable and compassionate Community Health Worker to support our
growing cardiometabolic program. As part of our clinical care team, the CHW supports Flyte
Health patients by collecting weekly and monthly biometrics, providing virtual and telephonic
outreach to enhance patient self-management, care navigation, program adherence support,
and referral management. This role acts as a non-clinical partner to patients, helping them
overcome obstacles, understand their care plans, and stay engaged with the FlyteHealth
cardiometabolic program.
This position is ideal for someone who is compassionate, patient-focused, and comfortable
engaging by phone, video, SMS, and digital tools and has worked within Athena EMR.
Key Responsibilities:
- Build trusting, long-term relationships with cardiometabolic patients using culturally aligned, strengths-based communication.
- Support training and collection of biometrics - weights, BPs, labs
- Support appointment reminders, Rx filling and patient escalations
- Conduct telephonic or video check-ins to assess progress, challenges, and barriers to care.
- Reinforce care plans created by Flyte Health clinicians (e.g., weight scale usage and monitoring and blood pressure monitoring, device usage, visit schedules, nutrition or activity goals, advocating patients needing any outside referrals).
- Support patients in developing routines and confidence related to self-management.
- Identify signs of disengagement or rising risk and escalate proactively to clinical team members.
- Follow up on referral completion and address barriers such as insurance, transportation, or technology challenges.
- Coordinate warm handoffs to nurses, care managers, or providers when clinical needs arise.
- Help patients understand their cardiometabolic treatment plan and what to expect across their care experience.
- Assist with appointment scheduling, reminders, follow-ups, and visit preparation.
- Guide patients through the use of Flyte’s telehealth platform, remote monitoring devices, and digital tools.
Inbound Patient Support (Phone & Digital):
- Respond to patient phone calls, portal messages, and SMS inquiries within scope, including:
- General questions
- Program support
- Device troubleshooting
- Reminders and instructions
- Non-clinical questions
Qualifications:
Required:
- Certification as a Community Health Worker.
- High school diploma or equivalent; associate or bachelor’s preferred.
- 2+ years with experience in community health, patient support, health coaching, care navigation, or social services.
- 1+ years Telehealth Experience
- Experience with Athena EHR required
- Strong empathy, communication, and motivational interviewing skills.
- Comfort with telehealth tools, apps, digital workflows, and patient-facing technology.
- Ability to work independently, manage a caseload, and build rapport over phone/video.
Preferred:
- Experience with cardiometabolic populations (diabetes, hypertension, obesity, lipids).
- Experience in a virtual-first or digital health environment.
